The Gemara (Berakhot 31a) establishes the foundation: one who prays must focus his heart toward Heaven, deriving this from the pasuk "direct their hearts, Your ear will listen" (Tehillim 10:17), and further learns from Hannah that prayer requires directing the heart specifically — not merely articulating words.

Building on that base, the Rambam (Mishneh Torah, Prayer 4:15) issues the sharpest ruling: a prayer said without kavana is no prayer at all, and one who prayed without it must pray again with kavana — defining kavana as clearing the heart of all thoughts and seeing oneself as standing before the Divine Presence.

The Shulchan Aruch (Orach Chayim 98:1) and the Tur (Orach Chayim 98) spell out what that inward focus requires: concentrating on the meaning of the words one utters, imagining oneself before the Shekhinah, and removing all distracting thoughts — reasoning by a kal va-chomer that one who would carefully compose words before a mortal king must certainly do so before the King Who searches all thoughts.

On the practical question of after-the-fact obligation, the Mishnah Berurah (Orach Chayim 98) draws a key distinction: praying merely to discharge an obligation rather than as genuine supplication before a King is wrong and one must be very careful about it — yet after the fact, one does not repeat the prayer, indicating that the obligation is technically fulfilled even without ideal kavana.

Rav Kook (Ein Aya, Berakhot 5:17) deepens the concept by distinguishing two dimensions of kavana: the particular kavana of understanding the meaning of individual words, and a broader kavana of the heart directed toward the greatness of God — the latter being what the Gemara's phrase "focus his heart toward Heaven" specifically adds beyond mere comprehension of the text.

Returning to the topic of preparation for prayer, the Sages taught in the Tosefta: One who prays must focus his heart toward Heaven. Abba Shaul says: An indication of the importance of this matter is stated in the verse: “The desire of the humble You have heard, Lord; direct their hearts, Your ear will listen” (Psalms 10:17). In other words, if one focuses his heart in prayer as a result of God directing his heart, his prayer will be accepted as God’s ear will listen. Rav Hamnuna said: How many significant halakhot can be derived from these verses of the prayer of Hannah? As it says: “And Hannah spoke in her heart, only her lips moved and her voice could not be heard, so Eli thought her to be drunk” (I Samuel 1:13). The Gemara elaborates: From that which is stated here: “And Hannah spoke in her heart,” the halakha that one who prays must focus his heart on his prayer is derived. And from that which is stated here: “Only her lips moved,” the halakha that one who prays must enunciate the words with his lips, not only contemplate them in his heart, is derived. From that which is written here: “And her voice could not be heard,” the halakha that one is forbidden to raise his voice in his Amida prayer as it must be recited silently. From the continuation of the verse here: “So Eli thought her to be drunk,” the halakha that a drunk person is forbidden to pray. That is why he rebuked her.

Rabbi Eliezer says: One whose prayer is fixed, his prayer is not supplication and is flawed. The Gemara will clarify the halakhic implications of this flaw. While praying, one must face toward the direction of the Holy Temple. One who was riding on a donkey should dismount and pray calmly. If he is unable to dismount, he should turn his face toward the direction of the Temple. If he is unable to turn his face, it is sufficient that he focus his heart opposite the Holy of Holies. Similarly, one who was traveling in a ship or on a raft [asda] and is unable to turn and face in the direction of Jerusalem, should focus his heart opposite the Holy of Holies.

Rabbi Yehuda HaNasi decreed a fast, and the Sages brought Rabbi Ḥiyya and his sons down to the pulpit to pray on behalf of the congregation. Rabbi Ḥiyya recited the phrase in the Amida prayer: Who makes the wind blow, and the wind blew. Rabbi Ḥiyya recited the next phrase: Who makes the rain fall, and rain fell. When he was about to say the phrase: Who revives the dead, the world trembled.

One Needs to Have Concentration While Praying, containing 5 s'ifim: One who prays needs to concentrate on the meaning of the words which one brings forth from one's mouth. One should consider [it] as if the Divine Presence is opposite one, and remove all distracting thoughts from one, until one's thought and intention remain purely about one's prayer. And one should consider [it] as if one were speaking to before a king of flesh and blood, and one were organizing one's words beautifully and concentrating on them so as not to stumble, all the more [one should do so] before the King of kings of kings, the Holy One, who is Blessed, who examines all thoughts. And so did the pious ones and the men of action, who would seclude themselves and concentrate on their prayers until they would transcend [their] corporeality and strengthen [their] mental power, until nearly arriving at the level of prophecy. And if another thought comes to one in the midst of the prayer, one should be silent until the thought is eliminated. And one should think about things that humble the heart and concentrate it on one's Father in Heaven, and not think about things that contain levity ("light-headedness"). When praying, you should be mindful that the Divine Presence is before you as it is said in Scriptures: "Pour out your heart like water before the Presence of Hashem." Arouse your concentration, and rid yourself of all disturbing thoughts, so that your thoughts and concentration are purely on the prayers. Consider, if you were speaking before a mortal king, how well you would organize your words and concentrate on them properly, taking care not to blunder. Certainly then, before the King, Who is King of kings, the Holy One, blessed is He, you must concentrate your thoughts before Him; because to Him, blessed is His Name, thought is the same as speech, and He scrutinizes all thoughts. Before praying, consider the exalted [majesty] of the Almighty, blessed is His Name, and the insignificance of man, and banish [the thoughts] of all humanistic pleasures from your heart. When praying you must concentrate on the meaning of the words that you utter with your lips, as it is said: "Guide their heart, let your ear be attentive." The Rabbis state that whoever does not recite prayer in the language of supplication—even if their heart is moved and stirred with desire to pray from inner feeling—has not fulfilled the foundation of prayer, which must be service of God, because they have directed their heart only to fulfilling their own inner emotion; rather, one must recognize that feeling is a proper means to arrange one's thoughts and spiritual state fittingly for prayer, but prayer's essence is loftier than all human emotions, for it is supplication before God to bring about His will, and therefore one must recite it in the language of supplication and envision how small the value of human emotions is compared to the exalted purpose of bringing about God's will through prayer.

ורבנן אמרי כל שאינו אומרה בלשון תחנונים. אפילו מצא לבבו מתרגש ומתפעל להיות חפץ בתפילה מצד הרגש הפנימי, מ"מ כל שלא ישים לבבו כ"א למלאות הרגשו הפנימי לבדו, אין זה יסוד התפילה שצריך שתהי' עבודת ד', וצריך . שיכיר שההרגש הוא אמצעי נכון להעמיד רעיוניו ומצב נפשו באופן נאות לתפילה, אבל עיקרה היא נעלה מכל רגשות נפש אדם כי היא תחנונים לפני ד' ית' להפיק רצונו. ע"כ צריך לאומרה בלשון תחנונים, ולצייר האמת כמה קטן הוא ערך רגשות לבב אנוש נגד התכלית הנעלה של הפקת רצון ה' ית' שבא ע"י התפילה.
