At the level of Torah law, a woman does not become impure through a bedikah finding alone unless blood emerged with a physical sensation, as the Rambam (Mishneh Torah, Forbidden Intercourse 9:1–2) states — though blood found internally during an examination carries the presumption that it was accompanied by sensation; and Rav Melamed (Peninei Halakhah, Family Purity 1:5) explains that blood exiting the womb without sensation is rabbinically treated as a ketam rather than full niddah blood, and the rabbinic decree applies only when conditions for transmitting tumah to a garment or vessel are met.

When a veset arrives, women are obligated to examine themselves, and the Gemara (Niddah 2a) establishes that a woman with a fixed veset who examines herself at that time and finds blood is tamei from that moment — her time of examination is sufficient; while the Gemara (Niddah 12a) adds that a woman without a fixed veset is forbidden to her husband until she examines herself whether asleep or awake, whereas one with a fixed veset need only examine herself while awake.

The Shulchan Arukh (Yoreh De'ah 184:9) rules that if the time of the veset passes and she did not examine herself and felt no sensation she is pure without examination, but adds in the name of a second opinion — which the Rema affirms as the accepted practice — that a woman with a fixed veset, or any woman on the thirtieth day, remains forbidden until she actually examines herself; and the Arukh HaShulchan (Yoreh De'ah 184:38) elaborates that this requirement to examine after the fact applies specifically where she has a fixed veset or the thirtieth day applies, whereas a woman known to have no fixed veset need not examine after the time has passed.

For a veset bedikah to render a woman tamei, the examination cloth must itself be checked and clean beforehand: the Shulchan Arukh (Yoreh De'ah 190:33) rules that when a woman examines herself with a cloth that was pre-checked and finds even a drop the size of a mustard seed — whether round or elongated — she is tamei; and the Arukh HaShulchan (Yoreh De'ah 190:87) clarifies that when the examination cloth was not pre-checked, the finding is treated like a ketam and requires the size of a gris and more to render her tamei.

Rav Mordechai Eliyahu (Darchei Tahara 2:37) rules that if a woman performed an internal bedikah with a cloth that was not pre-checked and a stain of exactly a gris is found, she is pure; if more than a gris, she is tamei — provided the cloth was stored safely after the examination; and Rav Mordechai Eliyahu (Darchei Tahara 2:36) rules that when she rubbed the cloth against her thigh after the examination, an elongated stain of any size renders her tamei while a round stain of a gris or less renders her pure, though he notes an opinion that is stringent even for a round stain.

Rav Ovadia Yosef (Maayan Omer 7:1:236) addresses the question of stain size and color in the context of bedikah findings, ruling that a stain smaller than a grain of barley is pure unless found immediately on a garment beneath her, and discussing the range of opinions on whether a black stain may be attributed to lice or other sources rather than menstrual blood.

If the woman was sitting in the bed and engaged in handling ritually pure items, and she left the bed and saw blood, she is ritually impure and those items are ritually pure. Although the Rabbis said that a woman without a fixed menstrual cycle transmits ritual impurity retroactively for a twenty-four-hour period, a woman with a fixed cycle counts her menstrual days only from the moment that she saw blood.

The Gemara further suggests: Come and hear the mishna below (58b): With regard to an examination cloth that was placed beneath the pillow and blood was later found on the cloth, and it is unclear whether it is the blood of an examination or the blood of a louse that was crushed beneath it, if the stain is round it is ritually pure. There is no concern that this blood might have come from her examination, as a woman examines through an act of wiping and a stain produced in this manner would not be round. And if the stain is elongated it is ritually impure, as this shape can be formed by an examination. The Gemara analyzes the mishna: What are the circumstances? If she sensed, then in the case where the stain is round why is it pure? Rather, is it not referring to a case where she did not sense? And yet the mishna teaches that an elongated stain is impure. This contradicts the opinion of Shmuel that a woman is rendered impure only if she sensed. The Gemara answers: No, the mishna is actually referring to a case where she sensed, but since she also performed an examination one might say this was the sensation of the examination cloth. Consequently, if the stain was elongated, as appropriate for a stain produced by an examination, the blood certainly came from her body, whereas if the stain was round it is pure, as this is not the usual appearance of a stain from an examination.

§ The Sages taught in a baraita: A colored garment renders a woman impure due to blood stains if she sees a blood stain on it. Rabbi Natan bar Yosef says: If she sees a blood stain on the colored garment she is not impure due to a blood stain, as the Sages enacted that women wear colored garments, and this decree was made only in order to be lenient with regard to their blood stains, i.e., so that they do not become impure. The Gemara questions Rabbi Natan bar Yosef’s use of the word: Enacted. The Sages enacted? What was their enactment? Rather, Rabbi Natan bar Yosef said the reason that the Sages permitted colored garments to women was only in order to be lenient with regard to her blood stains. The Gemara raises an objection: From the statement that the Sages permitted colored garments one can conclude by inference they were previously prohibited. But was there a time when it was not permitted for women to wear colored garments? The Gemara answers: Yes, as we learned in a mishna (Sota 49a): In the war [bapulmus] of Vespasian they decreed upon the crowns of bridegrooms, i.e., that bridegrooms may no longer wear crowns, and upon the drum [ha’irus], i.e., they also banned the playing of drums. They also sought to decree with regard to colored garments, i.e., that women may not wear such garments, but they said: This is preferable, that women should wear colored garments, in order to be lenient with regard to their blood stains, as a blood stain found on a colored garment does not render a woman ritually impure.

Rabbi Zeira said to Rabbi Abba bar Yirmeya: Does Shmuel mean that if she does not have a fixed menstrual cycle she is required to perform an examination, but if she has a fixed menstrual cycle she is not required to perform an examination? This is difficult, as the mishna teaches that even a woman with a fixed menstrual cycle is required to perform an examination before she engages in intercourse. Rabbi Abba bar Yirmeya said to him: The mishna means that if she has a fixed menstrual cycle and she is awake, she is required to perform an examination; if she is sleeping she is not required to perform an examination before intercourse, as this would involve much effort. If she does not have a fixed menstrual cycle, then whether she is awake or sleeping, she is required to perform an examination.

§ A certain elder asked Rabbi Yoḥanan: If the time of a woman’s fixed menstrual cycle arrived during her pregnancy and she did not perform an examination, what is the halakha? I raise this dilemma only according to the opinion of the one who said that the obligation for a woman to perform a self-examination during her fixed menstrual cycle applies by Torah law. What is the halakha? According to that opinion, one can claim that since the obligation of an examination during one’s fixed menstrual cycle is by Torah law, she is required to perform an examination even during her pregnancy. Or perhaps, since her blood has stopped, as a pregnant woman generally does not experience a flow of menstrual blood, she is not required to perform an examination. Rabbi Yoḥanan said to him: You learned the answer to your dilemma from a mishna (39a): Rabbi Meir says: If a woman was in hiding from danger, and the time of her fixed menstrual cycle came and she did not examine herself, nevertheless she is ritually pure, as it may be assumed that she did not experience bleeding because fear dispels the flow of menstrual blood. Rabbi Yoḥanan explains the proof: The reason she is pure is that there is fear, from which it may be inferred that in a case where there is no fear and the time of her fixed menstrual cycle arrived and she did not examine herself, she would be impure. Rabbi Yoḥanan concludes: Evidently, from the fact that Rabbi Meir rules that a woman is impure if the time of her period passed without a proper examination, he maintains that the obligation for a woman to perform an examination at the time of her fixed menstrual cycle applies by Torah law. And, nevertheless, since there is fear, her blood has stopped and she is not required to perform an examination. Here, too, in the case of a pregnant woman, her blood has stopped and therefore she is not required to perform an examination.

§ The Sages taught in a baraita: The presumption with regard to the daughters of Israel is that until they have reached their physical maturity they have the presumptive status of ritual purity, and adult women do not need to examine them to check if they are ritually pure before they handle consecrated items or teruma. Once they have reached their physical maturity, they have the presumptive status of ritual impurity, due to the possibility of an unnoticed menstrual discharge, and if they are still minors, adult women must examine them to check if they are ritually pure. Rabbi Yehuda says: They should not examine them by hand, because that is likely to scratch them and ruin their status, as it will be assumed that they are ritually impure with menstrual blood. Rather, they should smear them with oil inside and wipe them off on the outside. And through this method they are automatically examined, i.e., if at that age they are ready to menstruate, the oil will cause the blood to flow.

If she does not experience a physical sensation, but conducts an internal examination, and discovers bleeding within the vaginal channel, we operate under the presumption that it was accompanied by a physical sensation, as explained previously.

If a woman has a regular fixed period, then before the time arrives when she regularly perceives, she should insert a cloth so she will be sure (her menstrual flow has not begun), for otherwise, she must suspect that perhaps a drop of blood issued and disappeared without her being aware of it. If she failed to do so, and the time of her fixed period passed, she is forbidden to her husband until she examines herself thoroughly. If during the time of her expected period, she bathed herself, the examination will be of no avail. She must be stringent and consider herself to be in an unclean state, because it is presumed that the flux has come at its regular time. The bedikah of the cloth before relations must be checked immediately, even by electric light; however, the cloth after relations need not be inspected immediately following the bedikah and may be examined in the morning, and until daylight they are permitted to be together and even to have relations.

Even if a woman is definitively a niddah and not a zivah, she does not achieve purity through immersion unless she first stops seeing blood and performs a bedikah on herself, as derived from the Mishna in Niddah 68a—a woman who performed bedikah on herself achieves the presumption of purity, but if she did not perform bedikah she remains in a presumption of tumah—and the Rosh, Rashi, and Shev Yaakov understand that bedikah is required by Torah law as a necessary interruption in the status of tumah.

Upon waking from sleep, when the soul returns to the body, whatever the person's hands have touched becomes impure because an impure spirit rests upon them; therefore one should not take garments from someone who has not washed their hands to wear them, as that impure spirit attaches to the garments and defiles them, and this impure spirit has permission to dwell in any place where it finds a trace of its imprint.
